Biofilm-Mediated Immune Dysregulation in Chronic Pulmonary Disease Progression
Main Article Content
Abstract
Chronic pulmonary diseases, including cystic fibrosis, non-cystic fibrosis bronchiectasis, and severe chronic obstructive pulmonary disease, are characterized by progressive decline in lung function and persistent lower respiratory tract infections. A central driver of this pathology is the establishment of complex, three-dimensional microbial communities known as biofilms. These structured communities are encased in a self-produced extracellular polymeric substance matrix that provides profound resistance against both exogenous antimicrobial therapies and endogenous host immune clearance mechanisms. This paper explores the intricate relationship between biofilm development and the subsequent dysregulation of local and systemic immune responses. Through a comprehensive analytical framework, we demonstrate how persistent biofilm presence induces a state of chronic, frustrated phagocytosis, leading to persistent local inflammation and collateral host tissue damage. Furthermore, we investigate the cellular phenotypes associated with long-term exposure to biofilm antigens, specifically focusing on the exhaustion of adaptive immune populations and the polarization of innate immune cells toward regulatory or tolerogenic states. By synthesizing current literature with novel clinical analytical methods, this research elucidates the pathways through which biofilms orchestrate immune evasion and subversion. Ultimately, understanding these mechanisms provides critical insights into novel therapeutic targets designed not only to disrupt the physical structure of the biofilm but also to restore effective immune surveillance and clearance in the compromised pulmonary environment.
